ON Tuesday, January 23d, 1869, at 10 P. M., I was called to Mrs. J, multipara, in the last fortnight of her third pregnancy. I found her in bed, with occasional pains, and flowing. Enquiring as to her previous history, I learned that on the 7th instant she was taken suddenly with slight hæmorrhage, which, continuing for two days, became gradually so profuse as to oblige her to take to the bed. During this time she had more or less constant pain in the back. The hæmorrhage, which caused the patient no especial anxiety, at length ceased, and she experienced no return of it until the date above mentioned, when it re-commenced, accompanied by irregular uterine contractions, after each one of which she noticed an increased discharge of blood.

I at once suspected the existence of placenta prævia. By the touch I found the os uteri high up, undilated and undilatable. By this examination I could not determine the presentation. Abdominal auscultation located the foetal pulsations most distinctly to the right of the median line, on a level with and near the umbilicus. This fact, taken with the evidence afforded by external manipulation, convinced me that the presentation was pelvic. Not having any means at hand for the mechanical dilatation of the os I decided to run the hazardous risk of awaiting further developments. I accordingly enjoined the strict recum
